TOLEDO JUNIOR GOLF ASSOCIATION CODE OF CONDUCT PLEDGE, LIABILITY AND MEDICAL RELEASE
All players and parents/legal guardians must read the Pledge/Release agreement below.
Please note that membership in the TJGA is NON-REFUNDABLE and is a one year membership, annually renewable.

MEMBERS
I agree to adhere to the current United States Golf Association Rules of Golf© and etiquette of golf described therein. I will conduct myself in a courteous manner and in accordance with the TJGA Code of Conduct while on the property of the host venue. I also agree to follow all of the policies (including Dress Code) set forth by both the host facility and the TJGA.
In fairness to my fellow members, I agree to cancel all entries for any event in which I am unable to compete by following the TJGA Withdrawal Procedure.
I, hereby release the TJGA, its members, supporting organizations, directors, staff agents, escorts, site owners/hosts, successors and assignees from all liability, claims, or other forms of legal action, including costs arising there from, against any incident which results in or arises out of any loss, property or personal (death included), which may be sustained by the junior player member or parent/guardian during travel to, from, or participation in, any TJGA event or activity.
I understand that membership may be suspended or cancelled for willful disregard of the Rules of Golf, proper golf etiquette, or for dishonest, unsportsmanlike or unbecoming conduct.
I understand that I am agreeing to the use of my name and/or likeness for TJGA publicity and promotional purposes under NCAA and USGA guidelines.
PARENTS/GUARDIANS
In case of an emergency during an TJGA event, I grant my authorization and consent for a Supervising Adult to administer general first aid treatment for any minor injuries or illnesses experienced by the TJGA member. If the injury or illness is life threatening or in need of emergency treatment, I authorize the Supervising Adult to summon any and all professional emergency personnel to attend, transport, and treat the participant. It is understood that this authorization is given in advance of any such medical treatment, but is given to provide authority and power on the part of the Supervising Adult in the exercise of his or her best judgment upon the advice of any such medical or emergency personnel.
I understand that I am agreeing to adhere to any and all TJGA policies and procedures as they relate to parents/guardians, including, but not limited to, Dress Code and Spectator Policy and Guidelines.
I understand that I am agreeing to the use of both my name and/or likeness for TJGA publicity and promotional purposes under NCAA and USGA guidelines – as well as that of my child/ward.
